Obtaining the Application Forms

Application forms as per specimen below may be obtained from:

All Local Councils,

VAT Department - Ta` Paris B`Kara,

The Ministry for Gozo - Victoria Square Gozo and from

the Ministry of Finance and Economic Affairs - South Street Valletta.

Application forms together with all the required documents must be presented at the Ministry of Finance and Economic Affairs, Mondays to Fridays between 8:00am till noon or sent by post to the Ministry of Finance and Economic Affairs, South Street, Valletta.

Gozitans may submit their applications at the Ministry for Gozo, Victoria Square, Gozo.

Form 2

Form 1a and Form 1b - Used to claim the VAT on the goods and services rendered. Form 2 - Used to indicate those services for which no expenses have been incurred.
